As part of our work on the Diabetes project cards concept we are looking for a name for these cards! Some possible names are listed below - please join in and help to name a tool which can help to emphasise patient choice by helping them to take control of their care agenda. Final entries by 16th February!

I suspect amongst my patients that agenda cards will be popular because that is what they are - a list of matters to be discussed. (I also like the term because it comes from the Latin meaning things to be done, which would get some action from the clinician)...however I feel a more catchy title is needed! They are shopping list cards,consultation cards, inventory cards, hit list cards, wish list cards, index cards, diabetes log cards
I like DIAMOND cards (DIA -betes; MO -motivate; ND -and be in control).
How about "Lets make a deal" as this also fits in with the concept of agreed care planning which is the next step after agreeing agendas and identifying needs and possible ways forward, be they medical or non medical.
